Clinical Integration Solutions

A rapidly growing IPA serving more than 1,600 physicians in multi-specialty group and independent practices, along with a comprehensive academic medical center and a hospital system in the Midwest. The IPA has implemented MedVentive Population Manager integrating with a diverse set of physician practice management systems, pharmacy management systems, lab data, and electronic health records (EHRs).  By using transparent, trusted data, the resulting clinical integration program has aligned incentives and produced measurable improvements in the quality and coordination of care. In engaging MedVentive, the IPA had several objectives including being able to sign gain-sharing contracts with large employers, managing and improving care guidelines, implementing actionable analytics for quality and pharmacy, integrating work queues for efficient office workflow, automating patient outreach, and measuring and reporting on physician performance.

Physician Efficiency Reporting to Identify and Act on Quality of Care Improvements

A management services organization (MSO) serves as a staff model, medical group with more than 100,000 managed care, Medicare, and PPO patients; an independent physicians association (IPA) with more than 350 physicians and 30,000 patients; and a major medical center. The MSO receives and scrubs all its data sources (claims, lab, pharmacy, radiology) internally and is transitioning to an electronic health records system. The MSO engaged MedVentive with a primary objective of evaluating physician efficiency and effectiveness. It sought actionable, credible reporting, deep drill-down capability, risk adjustment for severity of illness, and easy access by various departments. The MSO is implementing MedVentive Risk Manager.

Pay For Performance Management and Risk Management

An independent physician association (IPA) in the Northeast serves more than 250 physician members affiliated with two hospitals.  The IPA physicians care for 40,000 lives under commercial, Medicare and Medicaid lines of business with capitated and incentive-based quality contracts. In engaging MedVentive, the IPA had several objectives including incorporating a large, full-risk Pay-for-Performance contract (P4P) and other risk programs, reducing pharmacy costs with interventions for providers and patients, deploying physician and patient quality interventions to increase publically reported scores and reduce gaps in care, improving profit margins through provider collaboration, and identifying opportunities for greater primary care physician and specialist efficiency. The IPA chose MedVentive Risk Manager as the foundation for managing its risk contracts. 

Integrating Small Physician Practices, Specialists for Population Management

A physician hospital organization (PHO) brings together a hospital and an independent physician association (IPA) of more than 700 physicians in the Northeast. The PHO holds contracts with a variety of health plans for 40,000 commercial lives and delivers medical management functions. The PHO partnered with MedVentive and implemented MedVentive Population Manager for its business and clinical objectives including proving their quality of care, implementing a fair and consistent reporting system for physician quality, achieving clinical integration for collective bargaining, and identifying a standard of care for preventive health screenings and chronic conditions.
