Central Ohio PHO Focuses on Clinical Excellence by Improving Patient Care and Clinical Outcomes
WALTHAM, Mass. – July 27, 2011 – Mount Carmel Health Partners in Columbus, Ohio, has chosen MedVentive Population Manager to support the group’s clinical integration initiative, and to help partner physicians and the Mount Carmel Health System improve patient care and clinical outcomes through quality improvement programs that can be tracked and measured via the solution.
As part of its clinical integration program, Mount Carmel Health Partners is implementing physician-directed quality improvement programs, focusing on improving patient care and clinical outcomes. All providers are accountable for delivering demonstrated value to patients, payers and the community. Mount Carmel Health Partners defines value as the quality, service and outcomes providers deliver, as compared to the overall cost of care. In turn, this quantified value is used to benchmark provider performance and determine pay-for-performance reimbursement.
“MedVentive Population Manager is the foundation for each of our participating providers to achieve the agreed-upon results and be viewed as value leaders in the Central Ohio community,” said Michele Helbig, Mount Carmel Health Partners’ executive director. “The MedVentive registries and metrics will allow our providers to be rewarded for clinical excellence.” MedVentive Population Manager supports Mount Carmel’s clinical integration and population management goals. With its point-of-care registries, the solution provides robust data acquisition and aggregation, provider and patient attribution, and physician quality reporting system (PQRS) submission. Care management workflow tools and patient outreach tools streamline provider tasks and patient follow-up communications.
With MedVentive Population Manager, a physician and his or her staff can now identify patients with a chronic condition, such as diabetes, and automatically do outreach to this group of patients that helps to drive care compliance and improve their overall health.
“We’re pleased to assist Mount Carmel Health Partners achieve their clinical integration goals, and maximize and document the value they provide to their patients,” said Nancy Brown, MedVentive’s chief growth officer. “They have done an amazing job designing their program and gaining physician buy-in and we have no doubt they will successfully improve clinical outcomes.”
“I feel that the Patient Registry is the backbone of our clinical integration program,” said Dr. Daniel Wendorff, Mount Carmel Health Partners’ medical director. “As we move to the future of healthcare delivery, we are going to need to monitor constantly and improve quality of care and outcomes. MedVentive Population Manager not only allows us to monitor populations but, with its point of care ability, it will allow our physicians and their staff to proactively address gaps in care as well as allow them to work off a common platform.”

While healthcare was evolving the concept of an accountable care organization, MedVentive was busy operating as one. MedVentive was founded as the Provider Service Network (PSN) in 1997 by the CareGroup Healthcare System, an integrated, at-risk network of 4,500 physicians and eight hospitals that cared for more than 450,000 at-risk patients. The network included the Beth Israel Deaconess Medical Center, a teaching facility of Harvard Medical School.
Our origins as an ACO give MedVentive a deep understanding of provider organizations and the healthcare environment. Having been an at-risk provider network, MedVentive is uniquely qualified to support an organization’s transition from a fee-for-service payment environment to risk-based contracting, and provide the clinical integration/population management infrastructure necessary to be successful under this new model.
